2026 Baseball 4-Day Camp

As the parent or legal guardian of the above-named student ("the Participant"), I hereby give my full consent for the Participant to participate in the Indianola Baseball Camp (the "Camp").

I understand and acknowledge that participation in baseball involves inherent risks, including but not limited to, sprains, strains, fractures, collisions, and other injuries that may occur while participating in drills, games, and related activities. I understand that Indianola Baseball Camp and its staff will take reasonable precautions to ensure the safety of all participants, but it is impossible to eliminate all risks.

In consideration for the Participant being allowed to participate in the Camp, I, on behalf of myself & the Participant, hereby release, waive, and forever discharge Indianola Baseball Camp, its owners, operators, employees, volunteers, coaches, and agents (collectively, the "Released Parties") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or relating to any loss, damage, or injury, including death, that may be sustained by the Participant while participating in the Camp , whether caused by the negligence of the Released Parties or otherwise. I further agree to indemnify and hold harmless the Released Parties from any and all loss, liability, damage, or costs, including attorneys' fees, that they may incur due to the Participant's participation in the Camp.

I represent and warrant that the Participant is in good physical health and has no medical condition that would prevent him/her from safely participating in the Camp. I will inform the Camp staff of any pre-existing medical conditions, allergies, or special needs the Participant may have by providing detailed information.

In the event of an accident or illness, I hereby authorize the Camp staff to administer first aid and to seek emergency medical treatment for the Participant if deemed necessary. I understand that reasonable attempts will be made to contact me in such an event. I agree to be responsible for all costs associated with such medical treatment.
