As the parent/gaurdian listed above, I agree to the following Participation Consent and Release

As the parent or legal guardian of the above-named student(s) (“the Participant(s)”), I hereby give my full consent for the Participant(s) to participate in the Indianola Dance Team Clinic (the “Clinic”).

I understand and acknowledge that participation in dance and related activities involves inherent risks, including but not limited to muscle strains, sprains, bruises, fractures, slips and falls, and other injuries that may occur during stretching, conditioning, choreography, performance, and related activities. I understand that the Indianola Dance Team Clinic and its staff will take reasonable precautions to help ensure the safety of all participants; however, I recognize that it is impossible to eliminate all risks.

In consideration for the Participant(s) being allowed to participate in the Clinic, I, on behalf of myself and the Participant(s), hereby release, waive, and forever discharge the Indianola Dance Team Clinic, the Indianola School District, their directors, officers, employees, volunteers, coaches, and agents (collectively, the “Released Parties”) from any and all liability, claims, demands, actions, or causes of action arising out of or relating to any loss, damage, or injury, including death, that may be sustained by the Participant(s) while participating in the Clinic or related activities, whether caused by the negligence of the Released Parties or otherwise. I further agree to indemnify and hold harmless the Released Parties from any and all loss, liability, damage, or costs, including reasonable attorneys’ fees, that they may incur due to the Participant’s participation in the Clinic.

I certify that the Participant(s) is/are physically able to participate in the Clinic and that I am not aware of any medical condition that would prevent safe participation. I authorize the Clinic staff to secure emergency medical treatment for the Participant(s) if deemed necessary, understanding that reasonable attempts will be made to contact me prior to such care whenever possible.
