Indianola Basketball Camps

As the parent or legal guardian of the above-named student(s) (“the Participant(s)”), I hereby give my full consent for the Participant(s) to participate in the Indianola Basketball Camp (the “Camp”).

I understand and acknowledge that participation in basketball and related athletic activities involves inherent risks, including but not limited to muscle strains, sprains, joint injuries such as ACL or meniscus tears, bruises, abrasions, lacerations, fractures, concussions, and other injuries resulting from slips, falls, or contact with other players, equipment, or the court surface. I understand that the Indianola Basketball Camp and its staff will take reasonable precautions to help ensure the safety of all participants; however, I recognize that it is impossible to eliminate all risks associated with competitive sports.

In consideration for the Participant(s) being allowed to participate in the Camp, I, on behalf of myself and the Participant(s), hereby release, waive, and forever discharge the Indianola Basketball Camp, the Indianola School District, their directors, officers, employees, volunteers, coaches, and agents (collectively, the “Released Parties”) from any and all liability, claims, demands, actions, or causes of action arising out of or relating to any loss, damage, or injury, including death, that may be sustained by the Participant(s) while participating in the Camp or related activities, whether caused by the negligence of the Released Parties or otherwise. I further agree to indemnify and hold harmless the Released Parties from any and all loss, liability, damage, or costs, including reasonable attorneys’ fees, that they may incur due to the Participant’s participation in the Camp.

I certify that the Participant(s) is/are physically able to participate in the Camp and that I am not aware of any medical condition that would prevent safe participation in high-intensity physical activity. I authorize the Camp staff to secure emergency medical treatment for the Participant(s) if deemed necessary, understanding that reasonable attempts will be made to contact me prior to such care whenever possible.
